I/We accept and will abide by the policies, rules and regulations of Deerstream.
I understand that enrollment is for the entire spring session (11 weeks), effective upon the submission of this application. I understand that tuition PER DAY is $100 for Cafe & Play only, $150 for Afternoon Class only, and $225 for Cafe & Play + Afternoon Class. I understand that once I have made a payment for my child, this payment is non-refundable.
If a problem arises with my child in the areas of misconduct, disruptive behavior, neglect of work, or failure to follow established policies, we understand that resulting discipline may include suspension or dismissal. The determination of the Deerstream Board of Trustees is conclusive in all matters of discipline. I/We agree to release and hold harmless Deerstream and the host church, their directors and agents, from all claims, damages, or other liabilities for injuries to children which are not a result of gross negligence on the part of the organizations. I/We also agree to reimburse Deerstream and/or host church for any property damaged, destroyed, or lost as a result of intentional or negligent behavior of my child. Deerstream reserves the right to use photographs of the student in Learning Center Publications. Parents may restrict this right by advising the Executive Director in writing within 10 days of the execution of this registration form. This agreement may be deemed null and void by Deerstream if the required minimum number of students per class is not met. In such event, tuition deposits, supply fees, and payments made will be fully refunded. If a registered student voluntarily withdraws, there will be no refund. |
